Genderbending?

Maybe Naruto swapped genders with Tsunade. There are a couple of options with this swap.

One is a physical gender swap, where Naruto is now physically a female while Tsunade is physically a male. They are going to be treated as their new genders and their wardrobes will change into feminine (for Naruto) or masculine (for Tsunade) versions of their old clothes.

Option Two is a perceived gender swap, where Naruto is still physically a male, but is now seen by everybody else as a girl. Similarly, Tsunade will be seen as a male while still physically being a woman. Their clothes will change to match their new perceived genders.
