The tension is obvious in their body language - the way they hold each other's hands so tightly. You can tell this is a difficult topic to discuss, and feel the need to set their minds at ease.
"Before you begin, I just want to say that you can talk to me about anything - no matter how embarrassing you might think it is, I won't judge. Sam, you're my brother, I love you, and if there's anything I can do to help you through this process, I'll do it, no matter what."
Angelina smiles. "It makes us so happy to hear that. Sam had reservations about bringing this to you, but I feel it is important if she's to truly become the woman she was always meant to be." She gives his hand a hard squeeze and takes a deep breath. "I mentioned the side effects of the treatments she has been going through - the hormones, the breast injections. While they're perfectly healthy, prolonged use means that for the last few weeks I've been unable to satisfy Samantha.... sexually."
"Oh." That takes you back a little. "I'm, uh, sorry to hear that."
Sam has gone bright red - not just his face, but the crimson flush running down his neck and over the top of the swollen breasts within the tight white dress. Angelina presses on. "Samantha, no matter how hard we try, is not able to achieve or maintain an erection for any length of time. The hormones have caused his penis to shrink, to atrophy. It is so small he's barely able to pee standing up. I keep telling him it is beautiful of course, but beauty doesn't exactly help satisfy his... urges. For all the work the hormones have done to remodel his body, they have done nothing to ease his 'male instincts' shall we say. Instincts that I am no longer equipped to satisfy. Unfortunately there is too little left down there for me to work with."
Her hand slips around Sam's shoulders and for a fleeting moment you catch a strange tension between the two. Something about the slightly tight way her hand grips his bare shoulder, or the way he seems to shrink away from her. But then the moment passes and it is gone.
"The very same urges that got him into this situation in the first place, when I caught him in bed with another woman. Now I worry that, if I'm unable to satisfy them, he may turn to another who can. He may break our marriage vows for a second time."
"I'm sorry to hear that, but I don't see how I can help in this situation."
