The trusty old method of having a Device that can swap people's body parts, with the choice of whether or not the swapped individuals are aware of what had happened to them or not. Maybe only the device wielder is aware of the swaps? Your choice.

Another popular way of part swapping is by someone having a Power to exchange people's parts (again, with or without their awareness). It could be an innate power that surfaces, or maybe it's gifted to someone by someone or something.

Maybe it can be the Role Exchanger, an alien entity that experiments with humans by swapping them around in various ways. This is also a well known and popular method of swapping, with everyone swapped aware of their changes and with no power to stop the entity.

Perhaps one of the characters can Possess other people's bodies and can them Swap the body parts of the people he/she has possessed. This isn't purely part swapping but has a lot of promise.
