So sorry to change horses in mid-day, but….my poem is actually a Senyru, not a Haiku. I have revised my poem after receiving feedback and revised the form information. Nursing's documentation is really driven by patient needs and patient care that is provided. The nursing note is a legal document that is evidence of the care that the patient receives. These days, with the advent of the electronic health record (EHR), it is easier for nurses to document and through this mechanism, we can capture and quantify the work we do that constitutes the profession of nursing. The primary focus, however, is still competent nursing care. I wanted to poem to reflect this. But the axiom still holds true...
