Question about birth certificates and adoption

This is not exactly about writing but I need info for something I plan to write.

What happens to the original birth certificate when a baby or a child is adopted by a couple?
Is the original birth certificate with the biological mother's name given to the adopting parents or do they issue a new birth certificate with the adopting parents' names?
