Along with what NW noted in her reply, the template system in the tool adds confusion to the average member on WDC that just wants to send a simple review. The tool is really intended for power users who review a lot.
I added a new window icon to the "Review This Item" floaty that opens the review tool. I think it's pretty nifty now.
