When a reviewer reviews a book entry, the review does indicate which entry the review was for. The only time an entry is not indicated on a review is when the reviewer reviewed the item as a whole. For example, viewing the reviews for "Invalid Item" , the most recent reviewed the item as a whole, but on 12-24-12 @ 11:23am, it received a "Review of Entry: "June 16 - Glass"
[As an update, I see the entry isn't shown when the review is linked by the {review} tag... we'll get that fixed; that is the only place where the entry info doesn't seem to be displayed.]
