Taking notes huh? For a class? I suggest going through the syllabus and basing your outline on the chapter that will be covered so that your headers and subheaders will be in place. If the teacher provides powerpoint slides or something before lectures, use those. If nothing else, it will be less writing for you in class and will get you familiarized with the material.
I also recommend going to Purdue Owl (and not just because I'm a Purdue grad either). Do a google search for "Purdue Owl Outline" there are lots of samples for various types of outlines.
