I also think that doing things expresses love and caring for someone. They can be small things. For example, I'll bring my husband a glass of tea or refill it when I go into the kitchen. I'll do something for him. In my house he's the one who usually washes the dishes. (We don't have a dishwasher.) So sometimes he'll come home and the dishes are done.
I don't know if this gives you any ideas, but I hope it sparks something in your mind.
Definitely keep that communication open and honest. I think Charlie ~ is right, to show improvement helps people to understand that you're really trying.
One more thing I would suggest is finding a good therapist. I've done that in the past and have been pleased with the outcome.
