Yeah, it's good that you found the way you can be in charge over her by realizing she doesn't want her door closed. It would be great if you can find out how to have her wearing briefs. At the ship, we dealt with all sort of assholes just by listening to their whining and talking to them but I realize your situation is extreme and conversation with her wouldn't help at all. My cousin is working in a nursing home in Austria and I know she has extreme patients like that, too. She has ill thyroid herself and because of her own health, she's unable to keep up with those extreme patients. I think she does the same as your leading nurse suggested to you; to be strict and literally don't give a fuck about their tantrums. Sometimes she let them laying in their piss and shit until they call her to change it. Then she gets even angrier but still she has to do it. But none of her patients changed their behavior no matter what she did. It did affect her though because no one is a stone-cold of course. For this reason, don't lose yourself in that job. On the ship we have a rule that says: work smart, not hard. Because no one will give you praise and medals because you broke yourself physically and mentally for someone else.
