"Somewhere in the Midwest" pretty much means Ohio and probably somewhere closer to southeast Ohio since that area is a bit more rural but would be around the Appalachians. It'd also mean that Saratoga Falls is by far the largest city in that area of the state since all of the other larger cities are on or close to Lake Erie minus Columbus (center of the state) and Cincinnatti (southwest on the Kentucky border).
As far as other Midwestern states go, Indiana, Illinois, Nebraska, Iowa, Kansas and Missouri aren't close to any particular mountain ranges AFAIK. Michigan, Wisconsin and Minnesota seem too far north/cold and again, lack of mountain ranges. That would leave the Dakotas and those are way too sparsely populated. After that you're getting into the mountain west which is, again, mostly sparsely populated.
So going by all of that then east Ohio/west Pennsylvania (if you wanted to stretch the definition) are the only places that make sense.
