Gotta Stick Together

You can't trust anything on the island, just looking at Adam and his new 25 or so pounds add tell you that so you decide you need a guinea pig.
"Adam!" you call down peeking over the edge.
"Uh?" says Adam looking up and seeing you.
"There's a ladder over there, hurry up and get up here!" you instruct and point towards the ladder.
Adam nods and heads towards the ladder. You look down at him while he climbs and see that his new weight and bigger butt has made his pants much tighter.
"I'm glad I found you," you greet as he climbs closer," Us boys have to stick together."
"Thanks Ian," Adam says huffing a little as he reaches the top of the ladder.
"What happened to you?"you ask faking some sincerity and poke his new belly.
"I ate some weird pink fruit," Adam tells you looking down at his gut.
With that you tackle him to the ground and sit on his thighs and hold his arms down," Sorry man, I gotta try something."
Adam struggles to get up, but you easily over power the smaller chunkier guy. You grab one of the faded blue cans and open it up and instruct Adam," Drink."
Adam refuses at first until you make him open his mouth. You also feel something brushing up against your leg. You realize that Adam is kind of turned on by the situation. You pour the drink down Adam's throat and give him a second of breathing time before pouring the rest down.
Adam coughs and struggles to get free.

What happens next?
