Shelly was born different, one of the rare cases of fetus-in-fetu in which she was born with her twin sister Ellen inside her womb. The medical community decided that it was safer to leave Ellen inside Shelly and keep a close check on their health.
Shelly was always noticeably rotund, but eventually (or occasionally) Ellen would have a growth spurt.
Karen was always pestering her parents for a baby sister, not knowing that her mother was not able to carry another child. After one such request from Karen her mother snapped and suggested that if she wanted a sister so badly, then she should carry it. Her father thought this to be an interesting idea, and knew the right people to make it happen.
