A few options

Nathan was going to stay with his uncle for the summer, the man was loaded. But also very out of touch. His estate hadn't changed from the 1940s, nor had any thing else. So uncle mistook regular teenager laziness, slackery and low grades for a bigger issue. It's how Nathan ended up in a velvet and lace suit complete with a cloth diaper and being pushed in a wheel chair down the hall. Uncle didn't want anything bad to happen to 'his precious simply nephew'.

In a simlair vain we have Chuck. A very lazy high school student who sucks his thumb when nervous. Chuck is made to take a test with a doctor and when told they won't affect his grades completely fails it. Now he wears jumpsuits and diapers to school as a part of a uniform for Dr.stevenons "special class".
