Great Big Parallel Universe: The boy is transported into a world much like his own, but it is inhabited by giant/normal-sized women and men who are as small as their thumb, pinkie-sized to the boy. The boy will have to survive his gigantic parallel family and/or their enslavement while appearing as a bug to their eyes.
An Anthropomorphic World: The portal leads into a different world where it is inhabited by anthropomorphic animals, but the portal is tiny on the other side, so he automatically shrinks The anthropomorphic animals live in lifestyles similar to those of humans (reminiscent of Zootopia), but humans do not actually exist. The boy must survive in this world where even a pygmy marmoset (the smallest mammal in the world) stands as tall as a two story building.

A Spooky, Monstrous World: The portal leads into a world where it is inhabited by monstrous giantesses, such as vampires, mummies, werewolves, gargoyles, or anything else from various monster movies or mythology with similar lifestyles to humans. The boy, shrunken to the size of one inch, is too frightened by the appearance of the monster giantesses to realize who is docile or hostile towards the tiny human kid.
