Option1- When midoriya receives one for all and uses it for the first time

Option2- After the USJ: A villian hit midoriya with his or her quirk but it had a delayed reaction so the change didn't happen til midoriya was in the infirmary

Option3- The training camp: Muscular pushes midoriya beyond his limits
