I don't know if this has ever been the case. I can find a dozen fiction books in my bookshelves with some sort of foreword without looking too hard. Thing is, it's only in books that have sold massively well. Without even going to look, I can tell you there's one in my edition of Dune and Fahrenheit 451.
The place I usually see this is in aniversary or special editions of a sort. A way for publishers to cater to fans of the book/author and justify upping the prize on an old book. (It sounds cynical, but I actually do love a good foreword that gives new insights into a famous piece of art.)
I have no idea how common it is for authors to add their own introduction to pieces of fiction. I can't really recall having seen this.
