You are Alex, a teenage boy in high school who wants to be a doctor some day. You have decided that you want to dominate and humiliate your little sister. You still love her, but she is such an easy target.
Your little sister, Nikita (or Niki for short) is a few years younger than you. She is still in middle school. She has always been a very short, skinny, and sickly girl, so she is thin and frail. Since she's home sick from school so often she has no friends. Mom and Dad work all the time and don't get home until late at night, so You're often the only one caring for poor little Nikita.
Our story starts with you in your room reading a book about theories on what causes cancer. You heard a knock on your door.
"Come in." You said.
It was your little sister Niki.
"Alex... I don't feel so good." The little girl said.
You could hear her panting. She sounded out of breath. You set down your book.
"Niki, you never feel good. Is this just your normal problems or is it something that matters?" You said.
"I... I'm having trouble breathing." Niki said.
"Ugh, fine. I'll give you a checkup. Take off your clothes." You said, grabbing your stethoscope and thermometer.
Even though you weren't a doctor (yet), you knew enough that your parents trusted you to correctly figure out what was wrong with Niki and if it was serious enough to take her to the real doctor. Niki sat down on your bed and removed her shirt. She still had a training bra underneath. She was so skinny and scrawny you could see her ribs.
You were rather ashamed of it, but your little sister's semi-undressed body was doing stuff to your dick that it shouldn't. You decided to make her take off more. You put your hand on her pants.
"No, not just the shirt. I need you naked so I can take your temperature." You said.
Niki shivered.
"B... Big bro..." She said.
"It's so I can take your temperature, silly!" You said, getting out a rectal thermometer.
Niki frowned. She hated the rectal thermometer, but that was what you always used on her. reluctantly, Nikita took off her pants, her training bra, and finally her cute little white panties. She got down on all fours for you.
"Please be quick..." She gasped.
You lubed up the rectal thermometer and slowly pushed it into Niki's butt. Niki probably thought you were trying to be gentle by going slowly, but really you were savoring every discomforted groan and quiver the girl let out. Naked Niki moaned.
"Are you done yet?" She whimpered.
"Almost." You said.
The thermometer beeped, indicating it had picked up a reading. Niki did in fact have a fever.
"Oh, Niki, you're burning up. You've got a fever." You said.
You slowly, gently removed the thermometer and put it aside so you could clean it later. Naked Niki sat back down as you got out your stethoscope. You listened to her chest, and her heart sounded fine. You listed to her tummy and heard only digestive noises.
"Did you eat anything bad for you?" You asked.
"No, just a couple graham crackers..." Niki said.
You went and listened to her back and heard things sloshing around. You realized that she was having trouble breathing because she needed to cough something up.
"Nikita, I found the problem... and I think I know how to solve it." You said.
"Really?" Little Niki said, smiling.
"Yeah! You just need to cough up some mucus." You said.
Niki gave you a hug.
"Oh, Alex, you're the best!" She said.
Nikita got out a tissue and tried to cough, but nothing came out. She kept trying to cough, but was unable to get anything.
"Oh... Oh no... It's not coming out..." Nikita moaned.
You patted your sister on her bare shoulder.
"Fear not, Nikita! I know just what to do!" You said.
You knew of three methods that really would work, but they would be very unpleasant for Niki... which was what made them perfect.
