Option 1: Alternative servant summoning: Shirou summons a servant other than Saber Artoria, and not being very experienced at being a master, and being passive, they make use of that to run rampant and make changes to the world and people around him. EX: Kama, Kiara, Morgan

Option 2: Fae Nature: Shirou having Avalon in him for so long creates changes in the world around him, allowing the Fae to come through and mess with the women and world around him.
