Option 1: Shirou did not make it out of the fire unscathed. Avalon kept him from getting truly corrupted, but it added a mental need to corrupt other people and made it so that anyone he touches gets corrupted. This was not noticed until Sakura met him as Kiritsugu was immune because his corruption left him dying and he never touched Taiga until after he met Sakura. (EX: Touching Sakura makes her more willing to take part in mana exchanges with Shirou and also more aggressive about what she wants/ Touching Saber results in her slowly altering into her Lancer Alter self that is loyal to Shirou and Angra Mainyu)
Option 2: Medea's husband: Shirou is the one that saves Medea instead of Kuzuki. He gives her a second chance at life and she takes it gratefully, becoming enamored with him; but she still has her fondness for changing people into her servants.
